As your primary care provider, your continuity of care is of the utmost important to us. If you visit the emergency room or an urgent care facility, please notify your TPCA primary care provider as soon as possible so that we can follow up with you to ensure proper transition of care.
After being notified of your visit to one of these facilities, our personable Transitional Care Management staff will reach out via phone to check on the status of your health. They will also recommend any follow up services, if needed, to ensure proper recovery.
